I consent to For Goodness Cakes to perform a background check, should that be deemed necessary for the type of volunteering I have indicated. I will be informed should a background check take place and the results of the background check.

I confirm that I have read and agree to the attached Terms and Conditions. I understand and acknowledge that I will apply the highest standard of food safety while baking and delivering donations on behalf of For Goodness Cakes. Failure to do so is considered gross negligence and not protected under the Good Samaritan Law.

I understand that if I do not complete the volunteer orientation, I will not be an authorized affiliate of For Goodness Cakes and therefore I will not be covered under their general liability insurance. I understand that if I do not comply with these policies, I could be suspended and/or terminated as a volunteer.

I am at least 18 years of age or older and am legally capable of signing the terms and conditions. I understand that if I am under 18 years of age that my parent/guardian must become the volunteer in For Goodness Cakes' system which means taking and passing the orientation. If I am under 18 years of age my parent/guardian is to supervise my accompaniment and is liable for any food safety issues should they arise.

I understand I will receive email notifications from For Goodness Cakes through Argenta the nonprofit management software used by For Goodness Cakes. I will white-list notifications@argentasoftware.com. I will not report any email coming from For Goodness Cakes or Argenta as spam. I understand if I do report email as spam I will no longer be eligible to volunteer for For Goodness Cakes.

I understand and acknowledge that if I am having symptoms of illness, COVID or otherwise, I will immediately inform my For Goodness Cakes chapter leader to have any cakes I have signed up for removed from my assignment, for a period of 30 days.

I understand and acknowledge that should I continue to bake and deliver without notification, I am held liable for gross negligence and will be banned from For Goodness Cakes indefinitely.
